As the parent of _______________________________, I have read the Internet policy for patron access (available at computer stations and the front desk of the library).  I understand that this access is designed for personal and educational research and that the library does not monitor or have control over content on the Internet.  I accept full responsibility for any damages my child causes to the computer or to the network.  I also accept full responsibility for my child’s supervision while he is using the Internet work station.  I will not hold the library or its staff responsible for any materials on the Internet that my child accesses, downloads, or prints.  I understand printed materials cost $.20 per page.

*All children and students under the age of 18 are required to have a Benton County Library card and an Internet Policy for Patron Access Parent or Legal Guardian Consent form on file.  This requires at least one parent have an active library card.

*The library affirms the right and responsibility of parents and guardians to determine and monitor their children’s use of the Internet.  Parents or legal guardians who believe that their children cannot responsibly use the library’s Internet access are requested to monitor their children’s Internet use.

*Children using the Internet are subject to the same first come, first served basis, with a thirty minute time period restriction if other patrons are waiting.

*The library Internet Policy applies to all patrons using the library computers.
