Honor Your Veteran

Please print this page, fill out the information listed below and return to the library for inclusion in the new pictorial history book of the Korean and Vietnam War Veterans.  You are welcome to include one picture also.  All pictures will be returned, please put your name and phone number on the photo.  All pictures will be printed in black and white within the book regardless of photo submitted.  This will be a treasured historical volume for generations to come.  We hope you will be proud of the addition of your family member to the book.
